If you would like to update your household information please use the forms below.
After completing Change of Information Form you can submit it with the following options.
NLG Change of Information Form
1) email it to us at admin@gitlaxdax.ca
If you choose this option please include your full name and detail information being changed in your email.
2) Print and bring completed form to GNTS during regular office hours.
Print your completed form and bring it to Gitlaxdax Nisga’a Terrace Society during regular office hours. Our staff will be available to answer questions and help fill out form.
PLEASE NOTE:
Name change updates will require documentation such as, a birth certificate, marriage certificate or name change certificate.
